Monday - Friday; 40 hours per week; Benefits Eligible
Service Coordinator is responsible for service coordination activities for each child on his/her case load. The Service Coordinator works closely with other team members in designing each child’s program and in monitoring progress. In addition, the Service Coordinator participates fully on the interdisciplinary team, assisting in decisions regarding individual children and program direction.
Education &/or Experience
- BS Degree or higher in health/human service or social work field required.
- Must have valid driver's license and be able to use personal vehicle, if company vehicle is unavailable.
- A minimum of three to five years of experience working with young children, preferably children with disabilities
- This position is subject to a criminal background check in accordance with the State of Alabama DHR guidelines
